CONSENT FORM TO USE PHOTOGRAPHS / MEDIA  -Adult Permission

Photographs and/or video recording may be taken at activities organized by St Peter’s Catholic Church. We would appreciate your permission to use images for information and display purposes. Anyone featured will not be specifically named in person. The images will not be used for anything which may be viewed as negative in tone or that may cause offence, embarrassment or distress for the child, parent, carer or adult. These images may be used in the church, the hall, Parish website, Diocesan newspaper (or similar), on Facebook (St Peters Catholic Church Gorleston) and on WhatsApp – in both social media cases, on a closed St Peter’s Church group - which you are all welcome to join, if you haven’t already. If you do NOT wish to give permission for specific use of photographs, please fill in only the child’s name on the form and your phone number, and state clearly in the empty space at the bottom of the form that you do not consent.
